Purpose
Momentum Tailor may record, photograph, or otherwise capture your likeness, voice, or participation during our courses, including in-person, live online, and on-demand digital courses. This media may be used for promotional, educational, or marketing purposes. By checking the box at the time of purchase, you agree to the terms of this Media Release Form.
Release and Consent
By agreeing to this Media Release Form, I grant Momentum Tailor, its affiliates, and authorized representatives the irrevocable right and permission to use my likeness, image, voice, or other identifying features (collectively, “Media”) captured during my participation in any course offered by Momentum Tailor, whether in-person, live online, or on-demand digital formats, for the following purposes:
- Marketing and promotional materials (e.g., website, social media, brochures).
- Educational or training content.
- Other purposes related to the mission of Momentum Tailor.
I understand that the Media may be edited, reproduced, distributed, or published in any format, including but not limited to print, digital, video, or audio, without further approval from me. I waive any right to inspect or approve the final materials.
Compensation
I acknowledge that I will receive no financial compensation for the use of my Media.
Rights and Ownership
I grant Momentum Tailor full ownership and rights to the Media for the purposes stated above. I confirm that I am at least 18 years of age and have the legal capacity to enter this agreement. If the participant is a minor, a parent or legal guardian must contact Momentum Tailor to discuss consent.
Opt-Out Option
I understand that I may opt out of this Media Release by contacting Momentum Tailor in writing at hello@momentumtailor.com before or during the course. For in-person events, I may notify staff to avoid being photographed or recorded. If I opt out after the course has started, Momentum Tailor’s normal refund and cancellation policy applies, as outlined at www.momentumtailor.com/refund_and_cancelation-policy
Liability
I release Momentum Tailor, its employees, and affiliates from any liability, claims, or damages arising from the use of my Media as permitted by this agreement.
Acknowledgment
By checking the box at the time of purchase, I acknowledge that I have read, understood, and agree to the terms of this Media Release Form.
